Position – Care Co-ordinator
Employment type – Permanent
Location – Chippenham
Salary – GBP28,000 p.a
Hours – Monday to Friday 37.5 per week

About the role – Duties will include:
Planning day to day rotas
Writing and updating care plans
Liaising with clients and families to support the needs of our service users.
Domiciliary care work as and when required
Covering the on-call phone service

Knowledge, Qualifications and Experience required
NVQ Level 3/Diploma in Health & Social Care.
Experience in planning staff rotas,
Experience in writing and maintaining client care plans
Experience in hands-on care support work.
Proven skills in the use of MS Office packages
Attention to detail and accuracy.
Strong teamwork ethic
It is essential that you have a full clean driving licence and your own car
