About the role – You will be supporting service users with access to the community, which will include employment, and educational opportunities. Aiding service users, in ensuring they are achieving maximum benefits and renewals to enable independence. Finally, you will be supporting service users in arranging and facilitating health related appointments and attending meetings on the behalf of the service user, and liaising with external professionals.

Qualifications and Experience required –

A driving license and access to own car is essential.
