Support Worker – Rough Sleeper Prevention Worker

Location: Exeter
Salary: £24,500 per year
Hours: 37.5 hours per week, Monday to Friday with occasional weekend work
Contract Type: Permanent

The Role
As a Rough Sleeper Prevention Worker, you will be at the forefront of tackling homelessness. You will support individuals to move from the streets into secure ‘off the street’ placements, including emergency sleeping pods. The role includes assertive outreach sessions, making contact with both new and long-term rough sleepers to assess their needs and provide person-centred support. You will be involved in delivering immediate assistance, conducting needs assessments, and creating personalised support plans to help individuals achieve positive outcomes.

Key Responsibilities

  • Carry out assertive outreach and visit identified hotspots to engage with rough sleepers
  • Manage referrals and provide emergency accommodation support
  • Conduct needs assessments and risk assessments for individuals accessing services
  • Empower clients to achieve personal goals by developing their skills, confidence, and wellbeing
  • Maintain accurate client records using the internal client record system
  • Participate in a rota system, which includes early mornings, evenings, and weekend shifts as needed

What You’ll Need

  • Valid UK driving licence and access to your own vehicle for business use
  • Experience working with vulnerable adults or those with complex needs and challenging behaviours
  • Strong relationship-building skills to engage clients and develop effective support pathways
